Ileostomy

Ileostomy is surgery to make a stoma on the abdominal wall which is connected to the end of the small intestine (ileum). This action is most often performed on inflammatory bowel disease or anal cancer.

A stoma created in an ileostomy can be permanent or temporary. A stoma is permanent when the surgeon removes the entire colon and anus. However, in an ileostomy, a temporary stoma is more often performed than a permanent stoma.

A temporary stoma in the ileum is created to give the inflamed colon or anus time to rest. Once healed, the surgeon will reconnect the ileum with the colon or anus, so that stool flow returns to normal.

colostomy

A colostomy is the creation of a stoma in the abdominal wall that is connected to the large intestine. Like an ileostomy, a colostomy can be performed to create a stoma that is either temporary or permanent. A stoma is permanent when the surgeon removes or cuts the lower part of the colon and closes the anus.

Meanwhile, a temporary stoma in the large intestine is performed to rest the affected part of the large intestine and rectum for a certain period of time. After the colon or rectum has healed, the surgeon will reconnect the colon so that stool can pass through the rectum normally.

Colostomy is commonly performed in colon cancer, diverticulitis, and fecal incontinence.

After the operation of making a stoma on the stomach, the surgeon will attach a special bag to the stoma, to collect the stool that comes out. Before the patient is allowed to go home, the doctor or nurse will teach you how to attach, dry, replace the stoma bag, and treat the stoma and the surrounding skin.

Poor stoma care can lead to complications in the form of skin irritation, especially in patients undergoing ileostomy. This condition will make the skin become blisters and sores, making it susceptible to infection. Therefore, it is important to know how to properly care for a stoma bag.

The duration of the use of the stoma bag

The stoma bag consists of 2 parts, namely a wafer and a plastic bag for collecting feces. Wafers are part of the barrier attached to the skin of the stomach, to protect it from dirt. When full, the plastic bag can be removed or replaced without the need to replace the wafer.

Wafers can be replaced when stool begins to pass between the wafer and the skin, or every 3 days to 1 week. The duration of the wafer usage depends on several things, namely:

  • How well does the wafer fit to the skin
  • Condition of the skin around the stoma
  • Patient physical activity

In addition to the stoma bag which consists of 2 parts, there is also a stoma bag in which the wafer and storage bag are combined. For this type of bag, the wafer will also be replaced when the storage bag is replaced.

Steps to replace a stoma bag

There are several things that need to be done when replacing a stoma bag, namely:

  • Clean the mucus that comes out of the stoma.
  • Clean the skin around the stoma with warm water and a washcloth.
  • Rinse the skin thoroughly.
  • Dry the skin around the stoma.

The skin can be cleaned with soap. However, choose a soap that does not contain fragrances and oils, as these can irritate the skin and make it difficult for the pouch to stick to the skin.

When changing the stoma bag, you need to check the condition of the stoma. Call your doctor right away if your stoma changes in size (significantly shrinks or enlarges), changes in shape (prolongs outward), or changes in color (pale, blue, or black).

In addition, you also need to check for bleeding from the stoma and signs of skin irritation around the stoma, such as redness or blisters. If you experience these things, contact your doctor immediately.

The operation of making a stoma on the stomach aims to divert the place of discharge of intestinal contents, namely feces or feces, from the anus to the abdominal wall.
